    ADDICT, ADDICT, ADDICT Words have power and they can hurt. I am well aware of this fact, and it is not my intention to cause anyone any more pain than they already have. But words also need to be precise and accurate and using gentle words to describe a harsh thing does not soften the harsh thing, it simply marginalizes the truth. So I have to ask, what’s wrong with using the word “addict”? The National Institute on Drug Abuse (NIDA) uses the term “addiction” to describe compulsive drug seeking despite negative consequences. Psychiatrists, on the other hand, prefer the term “substance use disorder” or SUD and that is the diagnostic term set forth in the fifth edition of The Diagnostic and Statistical Manual of Mental Disorders (DSM-5)—a diagnostic manual used by clinicians that contains descriptions and symptoms of all mental disorders classified by the American Psychiatric Association (APA). It’s become popular among some non-professionals to substitute SUD for addiction” and to refer to the “addict” as “a person with a substance use disorder.” In my opinion, this serves no good purpose. Good communication is never well served when seven words are used in place of one, but it is seriously wounded when the replacement words inaccurately describe the thing they mean to describe. The addict is controlled by a drug not a substance and drug or alcohol addiction is a brain disease, not a “disorder” and it needs to be treated as a disease. My wife has complained for years that I use too much dishwashing liquid when I do the dishes. Dishwashing liquid is a substance. Do I have a “substance use disorder”? Maybe I do, but it is hardly the same thing as being trapped in the clutches of heroin. And using the same term to describe my possible issue with dishwashing liquid as you would use to describe someone caught in the nightmare of heroin addiction diminishes the addict’s suffering and, more importantly, would allow others to minimize and marginalize the condition. Cancer is a disease in which some of the body's cells begin to divide without stopping and spread into surrounding tissues. Should we start referring to cancer patients as “people who have aggressive cellular growth”? How would this help us understand and respond appropriately to either the patient or the disease? I doubt there is a single cancer patient out there who would want us to drop the use of the word cancer in favor of the term “aggressive cellular growth.” But then, there is no stigma attached to cancer. No one blames the cancer patient for their disease or looks down on them as weak or lazy or having bad morals. There is an unfortunate stigma associated with drug addiction that makes it harder for many addicts to take the already difficult step of admitting their addiction and their need for treatment and causes many who are not in addiction to lack sympathy and empathy for those who are. But the cure for this is not changing the name of the disease. The cure is to treat and discuss addiction in the same way we treat and discuss cancer, as a serious disease that can be beaten with proper treatment. The NIDA has defined addiction as “a chronic, relapsing brain disease that is characterized by compulsive drug seeking and use, despite harmful consequences. It is considered a brain disease because drugs change the brain; they change its structure and how it works. . . It is as if drugs have hijacked the brain’s natural motivational control circuits, resulting in drug use becoming the sole, or at least the top, motivational priority for the individual.” We need to change the conversation about addiction treatment. We need to accept that Addiction is a chronic disease that rarely responds to a quick (30 days or less) fix. Research tells us that effective in-patient treatment leads to long term sobriety and fewer relapses. Ninety (90) day residential drug rehab is suggested as the minimum length of time for effective treatment. We need to provide for proper treatment of addiction. As most of you know by now, Anthony’s Act -- named for my 24-year-old son who lost his battle with heroin last Memorial Day despite three short-term stints at rehab. -- would amend the Affordable Care Act (ACA) to provide for a minimum of Ninety (90) days inpatient drug or alcohol treatment at a facility certified to provide such care by the appropriate regulatory body in the state in which it is located.
